Chapter 5 - Personal Medical Record

Balance makes it easier to communicate with your health professional by providing up-to-date personal medical records and detailed reports. When you select this feature, you see a list of reports that are gathered from your Calendar Events. The Personal Medical Record is a diary for your current health status and past medical history. To go to the Personal Medical Record, Click the Personal Medical Record icon on the Home Screen, or Click 'Jump / Personal Medical Record' on the menu bar.


  • Events


  • Events are simple graphs showing health events by time period. You can review All or individual types of Events Click the All button at the upper left. You can review: Tests; Symptoms; Immunizations; Doctor Visits; Hospital Stays; and Drugs. Click a selection to see a graph of the various events that have occurred over the past week, month, quarter, or year. For example, Click Symptoms in the Events column to review your symptoms over the past month.


  • Profile


  • You can select one of the profile forms listed here. fill in the information, and print it. These forms differ from the interview in that the interview helps you set up your calendar, targets and personal preferences, while the profile forms keep track of specific non-Calendar information by category. If you begin with the Personal Information, you can continue through to the Women's Health screen, filling in your profiles in one session. If you want to continue to the next screen, Click 'Next'. To go to the Contents screen, Click 'Contents' in the jump bar.
    Enter your personal profile information for any category: personal information; family history; emergency contacts; insurance data; conditions; or women's health. This information is used to create your Medical Record (described below) for use by you and your healthcare professional.


  • Print Medical Record


  • Click this button to print your personal Medical Record. You can use this when you visit your healthcare professional, or when you travel. It includes the personal health information entered in the Profiles and a summary of your health events including medications, tests, and immunizations, usually for the last six months.


  • Print Quarterly Diabetes Health Status Report


  • Click this button to print your Quarterly Diabetes Health Status Report. This report is ideal for your healthcare professional, as it provides detailed information about your health over the past three months. It includes: a list of the most important graphs you should print: a three-month review of your blood glucose test results, showing "In Range" and "Out of Range"; a three-month review of the drugs you have taken; symptoms you have reported during the past six months; and a three-month review of your exercise program.


  • Take Interview


  • This selection brings you to the Balance Interview. If you have not taken the Interview before, plan to take about 15 minutes to answer the questions and start planning better health management. The Interview asks important questions regarding your health. Read the instructions on the screen, and try to answer all questions (although it is okay to leave some answers blank). If you re-take the Interview, you will not see the plans that you have already scheduled from the Interview. Do not schedule them again, or you may see multiple entries in your daily Calendar. For more information about the Balance Interview, see item 5 of the
    BALANCE PC Quick Start in the chapter, Installation and Quick Start Information. After completing the Interview, Click 'Done'. You see the Home Screen next.
